Optimal Creatine Intake - Are you taking too little or too much?
Creatine is having another moment — this time framed as a brain supplement, not just a muscle one. You've probably seen clips of podcasters discussing doses in the 10-20 gram range for cognitive benefits. If you're already taking the standard 3-5 grams, it's a reasonable question: are you underdosing?
What they're saying:
Popular podcast clips have highlighted much higher creatine doses — sometimes 10-20 grams — as necessary to unlock cognitive and brain-energy benefits, distinct from the standard dose used for muscle and strength.
What the evidence actually shows:
The well-established, broadly supported dose for creatine's core benefits — strength, exercise performance, mood, and general cognitive support — remains 3-5 grams daily. A comprehensive review of creatine research across women's lifespans found this standard dose effective for strength and exercise performance in premenopausal women, with additional benefits for mood and cognition (Smith-Ryan et al., Nutrients, 2021).
That same review found something genuinely relevant to a 40+ audience: postmenopausal women may see added benefit — specifically to skeletal muscle size and function — from higher doses, around 0.3 grams per kilogram of body weight daily (roughly 18-20 grams for a 150-pound woman), particularly when combined with resistance training (Smith-Ryan et al., Nutrients, 2021). So the higher-dose conversation isn't pure invention — it has a real basis for this specific population and specific outcome.
Here's the most directly relevant piece: someone actually ran this exact experiment in healthy people. A 2023 randomized controlled trial gave healthy young adults either 10 grams/day, 20 grams/day, or placebo for 6 weeks, then tested processing speed, memory, and attention. The result: creatine had no significant effect on cognitive performance at either dose compared to placebo (Moriarty et al., Brain Sciences, 2023). This is the closest thing to a direct test of "does a higher daily dose sharpen a healthy brain," and in this trial, it didn't outperform placebo at all — let alone show a 20g-over-10g advantage.
That said, the broader picture on creatine and memory in healthy people isn't zero. A 2022 meta-analysis pooling 8 randomized trials found creatine supplementation did improve memory performance in healthy individuals overall, with the benefit concentrated almost entirely in older adults (ages 66-76); younger adults showed essentially no effect. Notably, the dose used across these pooled studies — ranging from about 2.2 to 20 grams per day — did not influence how much benefit people saw (Prokopidis et al., Nutrition Reviews, 2022). In other words, even where a real benefit shows up in healthy people, it's tied to age, not to how high the dose goes.
There is another interesting higher-dose brain study behind some of this discussion — but it comes with important context. A 2025 pilot trial gave 20 grams of creatine monohydrate daily for 8 weeks to 20 patients with Alzheimer's disease, and found it feasible, well-tolerated, and associated with an 11% increase in brain creatine along with measurable improvements on several cognitive tests (Smith et al., Alzheimer's & Dementia: Translational Research & Clinical Interventions, 2025). That's a real, notable data point for high-dose creatine and cognition — but it's a small pilot study (20 people) specifically in Alzheimer's patients, not a study of healthy midlife adults looking for a cognitive edge. The researchers themselves note that larger trials are still needed to establish efficacy and the optimal dose.
Separately, a different high-dose study looked at healthy adults rather than a clinical population — but under a specific stress condition. A single large dose of creatine (0.2 g/kg — about 14-18 grams) improved cognitive performance in healthy adults who'd been kept awake for 21 hours straight, reducing the deterioration in logical reasoning, processing speed, and vigilance that sleep deprivation normally causes (Gordji-Nejad et al., Nutrients, 2026). This is useful information in its own right, but it tested a one-time acute dose during severe sleep deprivation — not a daily brain-boosting protocol for a well-rested person.
Here's where it's worth pausing: both the Alzheimer's pilot and the sleep-deprivation study get cited in popular content as general evidence that "everyone" benefits from high-dose creatine for brain function — but neither one actually tested that, and the one trial that did test daily high-dose creatine in healthy adults (Moriarty et al.) found no benefit at all. One study is a small pilot in people with a neurodegenerative disease that already involves impaired brain energy metabolism, where creatine may be correcting a specific deficit. Another is healthy adults, but only under an acute, extreme stressor most people aren't experiencing on a given Tuesday. Extrapolating "20 grams helped Alzheimer's patients" or "a high dose helped sleep-deprived people" into “healthy 45-year-olds should take 20 grams daily for brain health" skips over the fact that a diseased or acutely stressed brain doesn't necessarily respond the same way as a healthy, rested one — and the direct test of that specific claim came back negative.
What this means for you:
3-5 grams daily remains a solid, evidence-backed baseline for the muscle, mood, and general cognitive benefits most people are after. If you're postmenopausal and focused specifically on muscle size and bone-supportive training, a higher daily dose has some research behind it — worth a conversation with your coach or doctor. If you're an older adult (66+) hoping for a memory benefit, standard-dose creatine has real support behind it too — but going higher than standard doses hasn't shown added benefit in the actual research, including the one trial built to test that question directly. The Alzheimer's and sleep-deprivation studies are genuinely interesting, but they don't add up to "healthy adults should take 20 grams for brain benefits" — that's a different question, and the closest direct test we have says no.
